Consumer Directed Services (CDS) caregiver/ PCA

**Job Summary**

**Responsibilities**

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Provide medication reminders and monitor vital signs in accordance with care plans and HIPAA regulations
• Support clients with meal preparation, cooking, and nutritional needs to promote healthy living
• Observe and document client conditions, behaviors, and progress to ensure safety and effective care planning
• Utilize patient observation techniques to identify changes in health status or behavior, especially for individuals with dementia, Alzheimer's, or developmental disabilities
• Operate assistive devices such as Hoyer lifts or wheelchairs safely during transfers and mobility assistance
• Maintain a clean and safe environment by performing light housekeeping tasks like cleaning and sanitation in accordance with health standards

**Qualifications**

• Prior experience working in home health, assisted living, nursing homes, group homes, or hospice care is highly preferred
• Knowledge of HIPAA regulations and patient privacy standards
• Certification in CPR and First Aid required or willingness to obtain upon hire
• Experience working with individuals with autism, developmental disabilities, dementia, or memory care needs is advantageous
• Ability to lift heavy objects safely and perform physically demanding tasks as needed
• Strong communication skills to effectively collaborate with clients, families, and healthcare professionals
• Compassionate attitude with a genuine desire to support others’ independence and well-being
