Job Description
- What we ask from you?
- -Working in a multi-disciplinary team alongside Clinical and Operational Leadership to ensure high quality care delivery to service users.
- -Provide specialist assessment and care planning in conjunction with service users and carers.
- -Evaluate treatment plans and undertake discharge planning and promote self care.
- -Be responsible for an agreed caseload.
- -Co-ordinate the work of the Team.
- -Support development of skills and competencies in others.
Essential Qualifications and training
Nursing Registration with the NMCDegree in nursing or equivalentMentorship/Assessor Qualification Desirable Qualifications and training
Specialist Practice Qualification in District NursingHealth assessment/ Leadership/ relevant modules
Essential experience
Community Nursing experienceLeadershipMDT workingCaseload ManagementClinical skills relevant to community nursing/ completed skills passport (inc LLA & doppler, compression & palliative care)
