Providing remote patient monitoring and support, the full-time Telehealth Nurse (LPN/LVN) will conduct regular phone visits to educate members on managing chronic conditions such as hypertension and diabetes while ensuring accurate documentation and fostering strong rapport with patients.

##### Key responsibilities

Perform regular telehealth calls to a dedicated panel of members

Provide medical, diet, and wellness education while ensuring accurate and timely documentation

Drive clinical outcomes through member engagement and coordination, escalating cases to the Provider team as necessary

##### Required qualifications

Current, active, unrestricted LPN/RN multi-state compact license in a nursing compact state

Minimum of 2 years hands-on nursing experience; telehealth and chronic care management experience preferred

EHR experience, specifically with AthenaHealth, is a plus

Fluency in English and Spanish is highly preferred

Reliable high-speed internet and comfort with navigating multiple computer applications
