Job Description
To support a transformative healthcare initiative, the full-time remote Multistate Licensed Nurse Practitioner will conduct telehealth visits, optimize patient medical therapy for chronic conditions, and collaborate with healthcare providers to enhance patient outcomes.
##### Key responsibilities
Conduct synchronous telehealth visits to assess and manage patients with chronic diseases
Order necessary lab tests and provide treatment recommendations to referring providers
Review remote patient monitoring data and achieve clinical outcomes for a panel of patients
##### Required qualifications
Minimum of 3+ years of experience treating patients with cardiovascular, cardiometabolic, or related conditions in an outpatient setting
Active multistate RN license and licensure as an Advanced Practice Nurse in at least 5 states
Completion of an accredited graduate-level APRN program
National certification as an APRN with an Adult, Adult-Gerontological, or Family focus
Current enrollment as a Medicare Part B provider
