Physical Health and Infection Prevention & Control Nurse - West Sussex

The post-holder will demonstrate an ability to undertake a physical assessment of patients with complex physical health needs, and will plan and support implementation of care and evaluate effectiveness.The post-holder may be required to develop specialist programmes for patients with complexneeds/conditions in collaboration with multi-disciplinary teams both within and external to SPFT to ensure that the needs of the patient are met.The post- holder will also demonstrate safe clinical decision-making and will provide expert advice, support and guidance to inform the safe and effective care of patients within a mental health/ learning disability setting.The post holder is expected to work collaboratively with PHIPC team members and SPFT clinical teams to meet the needs of patients and support the delivery of care objectives, quality improvement and the implementation of Trust policies and procedures as well as national best practice standards of care.The post-holder will participate in the development and delivery of training materials and resources to inform a high standard of physical healthcare/ IPC within SPFT services.The post-holder will have shared responsibility for management of a caseload of patients across SPFT with physical healthcare needs.
