Sexual Health Nurse - Training Post
Job Description
Work as part of the multidisciplinary team working
under the direction of line manager/Matron.
Assess care needs.
Develop programmes of care and implement and evaluate
these programmes of care for young people accessing Contraceptive Services.
The focus of
- the post holders role will be clinical with specialist young persons clinics, outreach visits, clinical review and administration relating to young people and child protection
- and core sexual health service provision.
- The post holder will actively try and engage young people who are
- unwilling/unable to attend mainstream services.
Undertake all relevant forms of care without direct
supervision and take charge in the absence of the person with continuing
responsibility.
Be expected to manage extremely sensitive situations.
Identify any child safeguarding issues and adhere to
the Trust safeguarding policies and procedures.
Identify those young people who are at high risk of
pregnancy and provide support and education with regards to contraception and
safer sex.
The
post holder will see clients on an individual basis, assess, plan, implement
and evaluate a programme of client care which is evidence-based utilising all
available resources and taking into consideration lifestyle, race, gender,
family and cultural background and ensure involvement, as appropriate with the
client, family carers and significant others. They will demonstrate competence
and sensitivity in undertaking a detailed sexual and social history to
establish the needs of the individual client and support, counsel or refer for
advice as appropriate.
Provide nurse led outreach sexual health clinics
- to young people in secure
- units, seeing clients who are often the most
- difficult and
- complex to engage in maintaining good sexual health.
Support the development of nursing practice within the
- specialist field through the use of evidence-based practice and in line with
- agreed national standards and contribute to the body of nursing knowledge.
- Provide safeguarding, young persons governance and follow up by means of
- a virtual
- diary for all young people where there
- are any concerns.
These concerns come from
counselling, nursing
and medical staff throughout the service.
Work as an autonomous practitioner within their sphere
of practice.
Continuously improve clinical standards in line with
clinical governance framework.
Exercise accountability as set out in the NMC Code.
Contribute to training and education within the
specialist area.
Provide specialist clinical advice and support to
patients and clinical staff within the Trust on the management and care of
patients within the specialist area.
Provide direct access for patients, either face to face
or telephone consultation Represent the service nationwide in the absence of
the line manager/Matron.
Play a key role in ensuring that national and local
standards/targets for the specialist area are met.
Comply with the Trusts vision, values and behavioural
compact.
