Be there for your community when they need you most! As a Social Worker in our Adult Social Care team, you will work within a dynamic Social Work Community Team, supporting individuals of all ages. You will use strength-based approaches to promote choice, control, and self-directed support, ensuring that people and their caregivers can manage their own needs and risks.

In this role, you will:

Assess individual needs and develop tailored support plans.

Promote independence and personalisation, enabling individuals to manage their own care.

Collaborate with health professionals and external agencies to ensure comprehensive support.

Uphold human rights and social justice, respecting the dignity and diversity of those you serve.

Engage in continuous professional development and reflective practice to enhance your skills.

To succeed in this role, you must have:

A recognised social work qualification and registration with Social Work England.

Experience in adult safeguarding and partnership working.

A full driver’s licence and access to a car for work-related visits.
