Work and Health Coach
Job Description
Job Purpose The
Work and Health Coach gives one-to-one, personalised support to people in
Wakefield who join the WorkWell programme. The role helps participants take
practical steps to return to work, or to stay in work, while managing a
physical or mental health condition.
You
will carry out work and health assessments and agree a clear action plan with
each participant. You will offer career and employment guidance, help identify
workplace adjustments, and refer or signpost people to health, wellbeing and
community services.
You
will manage your own caseload and keep accurate, up-to-date records so the
service can meet its targets and report on the difference it makes. You will
build good working relationships with GP practices, employers, jobcentres and
local voluntary and community groups.
Support
is delivered face to face, online and by phone, from a mix of home working,
local hubs and community venues across the Wakefield district.
For further details, see attached job description and person specification.
