Clinical Practitioner - Rangatahi

About the role
The Clinical Rangatahi Practitioner will provide high-quality, rangatahi-centred assessment, intervention and care coordination within Ngā Kete. You will work directly with rangatahi experiencing mental health, addiction, behavioural or wider wellbeing challenges. You will also work closely with whānau, schools, health providers and community agencies to develop coordinated and practical plans that reflect the voice, strengths and aspirations of each rangatahi. Alongside your clinical responsibilities, you will help strengthen the practice of the wider rangatahi team.
Key responsibilities
Undertaking culturally responsive clinical assessments and developing collaborative support plans.
Providing evidence-informed therapeutic and wellbeing interventions.
Supporting rangatahi experiencing mental health, addiction, trauma or other complex needs.
Identifying and responding appropriately to clinical risk and safeguarding concerns.
Working in partnership with whānau and recognising their central role in rangatahi wellbeing.
Coordinating care with schools, primary care, specialist services and community organisations.
Maintaining accurate, timely and high-quality clinical records.
Contributing to multidisciplinary reviews, case discussions and service development.
Supporting reflective practice and providing guidance to other team members when appropriate.
Helping develop effective clinical pathways, systems and practice standards for the rangatahi service.
About you
A relevant clinical qualification, such as social work, nursing, occupational therapy, counselling, psychology or addiction practice.
Current professional registration or recognised membership, where required for your discipline.
Experience working therapeutically with rangatahi and whānau.
Confidence completing assessments, managing risk and coordinating care.
Knowledge of mental health, addiction, trauma-informed practice and youth development.
A strong commitment to culturally safe and kaupapa Māori practice.
An understanding of Te Tiriti o Waitangi and its application within health and social services.
The ability to work collaboratively across clinical, cultural and community settings.
Clear communication, sound judgement and strong organisational skills.
A full New Zealand driver licence is required.
