About the role This is a varied, relationship-focused nursing role supporting clients with complex health needs in the community. You’ll have the autonomy to manage your caseload, backed by a team that’s there when you need them.

You’ll

• Assess clients and develop individualised care plans
• Coordinate care with clients, whānau and multidisciplinary teams
• Monitor progress and complete regular clinical reviews
• Provide clinical guidance, training and competency support to care staff
• Liaise with GPs, specialists, ACC, NASC and allied health professionals
• Provide clinical advice and support when more complex situations arise
• Build strong relationships with clients, whānau, referrers and our wider Life Plus team
• About you You’ll be a New Zealand Registered Nurse with a current APC and NZ nursing experience who enjoys working with people and making things happen.
