Job Description
Managing advanced cardiology caseloads, the full-time Registered Nurse Clinical Care Navigator will guide patients from diagnosis through recovery, ensuring seamless care coordination and operational success while working remotely.
##### Key Responsibilities
Manage a dedicated caseload of cardiology patients and facilitate referrals for testing and procedural appointments
Participate in the creation and evaluation of educational materials and clinical care paths to ensure adherence to medical plans
Collaborate with management on daily operations to enhance service delivery and patient satisfaction
##### Required Qualifications
Bachelor of Science in Nursing (BSN) from an accredited School of Nursing
Minimum of 5+ years of progressive clinical experience in Cardiology
Current and unrestricted Registered Nurse licensure
Proficiency in Evidence-Based Practice (EBP) and relationship-based care models
Ability to manage complex workflows and advanced patient scenarios
