Registered Nurse - Community & Home Care

About the role
Based in Penrose, Auckland, this full-time role offers the opportunity to provide clinical leadership and care coordination for clients receiving home and community-based support services. You'll work with people living with spinal cord injuries, traumatic brain injuries, neurological conditions and other long-term disabilities, helping them achieve the best possible outcomes. This position works Monday to Friday, 8:30am – 5:00pm, offering a great work-life balance with no shift work. While you'll be based in our Penrose office, you'll also engage with clients and support teams in the community.
Key responsibilities
Complete comprehensive clinical assessments and develop person-centered care plans.
Provide clinical oversight, guidance and support to clients and support workers.
Coordinate multidisciplinary services to ensure high-quality, seamless care.
Monitor clinical quality, risk management and client outcomes.
Support staff development through education, mentoring and best practice guidance.
Build positive relationships with clients, whānau, healthcare providers and external stakeholders.
About you
Current registration as a Registered Nurse with a valid New Zealand APC.
Experience in community nursing, rehabilitation, disability or primary healthcare.
Strong clinical assessment, communication and organisational skills.
The ability to lead, coach and support others.
Knowledge of complex care, neurological conditions and rehabilitation principles.
A genuine commitment to delivering person-centered care.
Cultural competence and the ability to work respectfully alongside people from diverse backgrounds.
A current full New Zealand Driver License.
