Job Description

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare or working on the Staff Bank within these organisations. Nursing and AHP vacancies are also open to Student Nurses and Newly Qualified Nurses/AHPs.

Join our Community Nursing Team supporting housebound patients across Thanet, including Margate, Ramsgate and surrounding coastal villages. Working within Primary Care Networks, you will deliver personalised care to patients with complex and long-term conditions, helping them remain safe and independent at home. We offer excellent development opportunities to build advanced clinical skills, including palliative and end of life care, medication management, IV therapy, wound care, catheter management, bladder and bowel care, and support for long-term conditions.

No two days are the same in this varied and rewarding role. You will assess, plan, deliver and evaluate patient care, undertaking activities such as wound assessments and dressing changes, administering medications, managing catheters, and providing palliative and end of life care. Working both independently and as part of a multidisciplinary team, you will organise and prioritise your caseload, maintain accurate clinical records, support colleagues, and respond to changes in patients' conditions. You will also play an important role in safeguarding adults and children, assessing capacity, escalating concerns appropriately, and working collaboratively with healthcare and multi-agency partners to achieve the best outcomes for patients.

Kent Community Health NHS Foundation Trust is currently undertaking a significant programme of transformation to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles. Depending on the area you are applying to work in, this could include potential changes to your role, work base, reporting arrangements, or terms and conditions of employment. Any changes would be subject to consultation and managed sensitively and in line with relevant employment processes. Please do contact the recruiting manager if you have any concerns about this so they can answer any questions you might have.
