About the role
The team supports with facilitating hospital discharges alongside supporting longer term rehabilitation goals, undertaking holistic assessments to optimise a patients ability and quality of life. You will form part of a team supporting individuals across a range of community settings including care homes, individuals homes and provision of strength & balance classes in community locations. You will support joint working with MDT colleagues as well as working as an autonomous practitioner, undertaking clinical problem solving and decision making. Other aspects of the role include supporting with triage and prioritisation of intervention, and appropriate delegation and supervision for band 3 and band 4 colleagues within the team.
There is a varied caseload including falls prevention, orthopaedic conditions, neurological and respiratory conditions. To be successful in this role, you will require a wide-range of clinical skills and community experience as well as . You will need excellent communication skills, adaptability and resilience to change. Lone working is required at times
The team is well supported with day to day Clinical Leadership as well as highly specialist therapists. There is a structured in-service training programme to help support and guide your development and opportunity to support students and apprentices.
There is close working with the urgent community response team and community nursing teams. This role also supports the cluster 8am 8pm across 7 days a week on a rota basis, within these hours to support both the intermediate care and UCR national pathways.
Working Pattern: 37.5 hours The service supports 7 day working on a rota basis

This is a fixed term contract from 22/11/26 to 21/11/2027. For current NHS employees this post could be a secondment opportunity.
