**Classification:** ZA7 - Clinical Nurse Consultant C Y1
**Salary per hour:** $66.08 + superannuation + generous salary packaging options
**Employment Type:** Fixed Term Contract Part-time 48 to 64 hours per fortnight until 9th October 2026
**Award:** Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028
**Role description**
Dhelkaya Health is seeking a motivated and experienced senior clinician to join our Community Nursing team as Complex Care Team Leader.
The Complex Care Program is a multidisciplinary service funded under the Health Independence Program (HIP). The program delivers hospital substitution and diversion services by supporting people with chronic and complex health needs in the community, ambulatory care settings and in clients' own homes. Our focus is on person-centred care that reduces avoidable hospital admissions and supports independence and wellbeing.
This is a senior clinical leadership role that combines team leadership with direct clinical care. The Complex Care Team Leader provides professional, clinical and operational leadership to the Complex Care team, ensuring high-quality assessments, coordinated interventions and safe, effective service delivery for community-based clients with complex needs.
The role also provides clinical expertise and leadership across the Continuing Care Directorate and works closely with Ambulatory Care, At Home and Community Nursing programs.
### **Key Responsibilities**
- Provide clinical leadership, supervision and support to Complex Care clinicians
- Maintain a clinical caseload while balancing leadership responsibilities
- Ensure delivery of high-quality, person-centred care and evidence-based practice
- Support hospital avoidance through effective community-based interventions
- Lead and contribute to quality improvement and service development activities
- Build strong relationships with internal teams and external stakeholders
- Act as a clinical resource within Dhelkaya Health and the broader community
**About you**
- Registered health professional with relevant tertiary qualifications
- Extensive experience in community, complex or chronic care settings
- Demonstrated leadership and team coordination skills
- Strong clinical assessment, decision-making and case management capabilities
