Health and Wellbeing Coach - West Somerset Primary Care Network

This is a predominantly patient-facing role. The postholder will deliver a regular programme of individual coaching appointments, reviews and, where appropriate, group sessions; manage an active caseload and use appointment capacity effectively.

Coaching may address long-term conditions,

prevention, emotional wellbeing, motivation, social isolation, healthy

behaviours, confidence, independence and the wider circumstances affecting

health. The postholder will work alongside social prescribing, care

coordination and clinical colleagues, recognising the distinct purpose and

limits of health coaching.

You will provide structured, person-centred coaching through face-to-face, telephone and digital appointments, selecting the format according to patient need, accessibility and service arrangements.

You will also use active listening, open questions and evidence informed behaviour change techniques and coaching tools to support patients to explore their priorities, strengths, challenges in readiness for change.

You will receive and respond to referrals through agreed PCN pathways, clarify suitability for health coaching and prioritise according to patient need, risk and service criteria.

Working flexibly across practices and community locations to respond to population need and variations in referral demand, rather than treating any single site or project as the exclusive focus of the role.

You will also proactively maintain an up-to-date knowledge of local community services, support opportunities, including those provided by Somerset Council, charities and voluntary, community and social enterprise (VCSE) organisations, to ensure patients are connected with appropriate resources that support their health and wellbeing.
