Full Time/Part Time/PRN Home Health Physical Therapist

Salaried Position of 85k-120k based on weekly points that fit your needs. Typical work week is Mon - Fri with opportunities to see patients on the weekends.

Job Description

Physical Therapist - Overview

• Assessing and re-evaluating the client’s condition and needs as required
• OASIS Start of Care for therapy only cases
• Provide patient care according to patient's care plan and needs
• Teach the patient and/or family members about care and support skills
• Communicate regularly with Nurse Case Manger, Clinical Care Coordinators, Interdisciplinary team, and overseeing physician
• Accurately and thoroughly document patient care within company policy
• Supervision of PTA
• Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life

Qualifications

• Licensed as a Physical Therapist in the state of Illinois with good standings
• Valid Driver's License and reliable vehicle with current insurance
• Current CPR Certification
• Prior Physical Therapy experience in a Home Health, Rehab, or Hospital setting is required
• Experience with home health Electronic Medical Records (EMR) software
• Comfortable with technology - a company tablet will be provided
• Strong communication and interpersonal skills
• Time management to balance patient care, travel, and documentation
• Great customer service
