In this role, you will:

Conduct assessments to identify individual needs and aspirations.

Coordinate a range of outcomes to meet those needs.

Promote personalisation and self-directed support.

Collaborate with health professionals and community partners.

Undertake safeguarding duties and risk assessments.

Provide support and guidance to individuals and their families.

Maintain accurate records and documentation.

To succeed in this role, you will need:

A recognised Social Work qualification.

Registration with Social Work England.

At least two years of post-qualification experience (for Band B applicants).

Knowledge of adult social care legislation and safeguarding practices.

Experience in managing complex cases and working collaboratively with health and social care partners.
