Job Description

Managing a roster of approximately 450 active participants, the remote Certified Lifestyle Coach will deliver structured coaching sessions focused on sustainable health and wellness behavior change for individuals with Type 2 Diabetes.
##### Key responsibilities:
Deliver structured coaching sessions via phone, chat, and group formats tailored to individual member needs
Utilize motivational interviewing and behavior change techniques to guide members in identifying strengths and setting realistic health goals
Coordinate care with Registered Dietitians, Nurses, and Providers as necessary to support member progress
##### Required qualifications:
National Board Certified Health and Wellness Coach (NBC-HWC) or actively pursuing certification
1+ years of coaching or behavior change experience with proven results
Experience providing virtual coaching in a healthcare environment regulated by HIPAA
Proficient in Microsoft Word, Excel, and Outlook
Demonstrated excellent written and verbal communication skills
