**Salary:** $62,354-$64,300/annually – based on experience

We are currently seeking a dedicated and passionate **Memory Care Coordinator.**

**Memory Care Coordinator Responsibilities include:**

• Supervise, develop, and schedule all resident care staff in memory care according to PSL policies, procedures, and standards
• Perform evaluations and reevaluations to determine appropriate residency, resident needs, and then assist in establishing appropriate service plans for memory care
• Orient and assist all residents in adjusting to life in the community in memory care
• Act as a liaison for all aspects related to resident care in memory care within the community
• Responsible for all documentation completed by resident care department staff in memory care, including 24-hour logs, care task charting, toileting schedules, and all other documentation
• On-call coverage as needed

**Memory Care Coordinator Education & Experience:**

• Licensed Practical Nurse (LPN) or Leadership in Resident Care experience, preferred
• An equivalent combination of education and experience in related field, required
• Compassion for and desire to work with the elderly, required
